SAN FRANCISCO, California—The Philippine Consulate General in San Francisco has released the yearlong schedule of its consular services in 2012.
Services to be offered are Philippine passport application, legalization of documents (affidavits, powers of attorney, deeds, other legal instruments), Civil Registry (reports of births, marriages and deaths), Dual Citizenship application and Registration for Overseas Absentee Voting.
Filipinos are advised to visit www.philippinessanfrancisco.org to download the forms and check the requirements.
- For passport applicants, please submit a copy of the data/photopage of the latest Philippine passport; and a copy of the US permanent resident card if available
- For Dual Citizenship applicants, please submit a copy each of the supporting documents (e.g. PH birth certificate/PH passport/PH marriage certificate, US naturalization certificate and US passport)
- For Reports of Births (for those whose fathers or mothers are Filipino citizens during time of birth), please prepare five (5) sets of the required documents.
- For Reports of Marriages (needed for the married name to be reflected in the PH passport), please prepare five (5) sets of the required documents, including copies of government-issued identification cards for both husband and wife.
- For legalization of documents, please bring the original document to be processed plus a photocopy of the document and two (2) copies of a government-issued ID.
Kababayans are also advised that the Philippine Consulate General only accepts cash, money order, bank draft, or cashier’s checks (made payable to the Philippine Consulate General) for the services rendered.
