Philhealth urges members to update record

THE Philippine Health Insurance Corporation (Philhealth) is requesting all its members to update their Member Data Record (MDR) to avoid hassles in claiming their benefits.

The member must fill out the amendment form M2 and submit it together with the photocopies of supporting documents.

In the declaration of dependents, the documents required are marriage contract/certificate with registry number for legal spouse, birth certificate with registry number for dependent children and birth certificate of parent and birth certificate of member for dependent parents.

If the parent?s birth certificate is not available, any one of the following IDs is acceptable: senior citizen?s ID, voter?s ID, driver?s license, passport, or a notarized affidavit of two disinterested persons attesting to the date of birth of parent to be attached to the Certificate of No Record found from National Statistics Office or Local Civil Registry.
